Without the patch, you'll just see Elwynn and Westfall as they always are. So no, you don't need a patch to access Elwynn and Westfall, only need if you want to see all the nice features and extra items the GMs spawned around the world. The patch is only about 800 kb, though.
Do note also that the patch in general is barely even a patch. It's just tossing an MPQ file into your folder. Considering that either way you can't retail and CotH from the same WoW folder, no damage as actually done. No special installer, no special loader [*cough*prolol*cough*]. Just toss the file in, and you never have to mess with it ever again.

Seriously, I don't want to use a patch to access Westfall or Elwynn Forest.

The patch is as easy to install as an add-on; its a single drag and drop file. I normally wouldn't inquire, but since it's a considerably easy task, can I ask why?

If there's an issue I'm not aware of with the patch and ease of access/compatibility I'd like to know. If it's just 'nope', then I'm not sure what exactly to say to that.
We change the realmlist to play here. This is seriously just as easy.

Nuhp! And I believe the patch is completely benign if you wish to play on other servers, as well, so there's really nothing to worry about with it save having to fumble a bit to find the folder.
